" Evolution of Nose Augmentation"

Originally, nose augmentation was done using the Closed technique, which had limitations in adjusting the structure, resulting in a less prominent nose. Currently, the Open technique is popular, allowing more structural adjustments, addressing various nasal issues, and achieving a more pronounced nose.

Materials Used in Nose Augmentation

Nasal Silicone

Medical-grade silicone, available in both custom shapes and pre-formed designs, such as Mantis or other silicone brands imported from the USA or Korea.

Artificial Tissue

Medical-grade collagen-based synthetic material that integrates well with the body.

Ear Cartilage

Cartilage from behind the ear is slightly curved, taken from the patient’s own body, providing durability and reducing the risk of a thin nasal tip.

Rib Cartilage

Rib cartilage from the 5th, 6th, or 7th ribs is strong and taken from the patient’s body. It enhances the nose bridge and creates a more prominent nasal tip while reinforcing the overall structure, reducing the risk of a collapsed nose in the future.

Common Nasal Issues

What is a Thin Nasal Structure?

Basic evaluation method

  1. Use the index and thumb to pinch the middle of the nasal bridge.

  2. Pull the nasal tissue to check its elasticity.
  3. If it stretches easily, there is potential for augmentation. If it hardly stretches, the nasal tissue is thin.

What is a Short Nasal Tip?

It can be observed by

  1. The nasal bridge length does not proportionate to the overall face

  2. Measurement from the top of the nasal bridge to the nasal tip
  3. Generally, the nose length should be about 1/3 of the face length. If the nose is shorter than this proportion, it indicates a short nose.

There are 3 types of nose augmentation techniques : Closed Rhinoplasty, Open Rhinoplasty with Ear Cartilage, and Open Rhinoplasty with Rib Cartilage.

Closed Rhinoplasty

A quick rhinoplasty technique using silicone or silicone with artificial tissue to support the nasal tip.

Closed: Ideal for those with a good nasal structure. Suitable for those not seeking a dramatic nose shape change and without structural nasal issues.

Open Reconstruction

Open reconstruction is a structural enhancement technique that involves exposing the nose to address a wide range of issues, from reducing the tip size to extending the nasal septum. This method effectively resolves concerns such as high humps, hooked noses, wide or short upturned noses, flat nasal bridges, and minimal tissue coverage. The results are refined, elongated, and naturally prominent. At Cosmacare Clinic, the silicone-free tip technique further ensures a seamless and lasting outcome.

Who is it suitable for?

This technique is ideal for those with structural nose issues such as a flat nose, crooked nose, bulbous nose, humps, or a nose with prior infections. It is also suitable for those seeking precise corrections. The procedure requires general anesthesia under the supervision of an anesthesiologist and typically takes 4-6 hours, depending on case complexity.

Ear Cartilage Rhinoplasty Technique

This method prevents nasal implant extrusion by combining customized silicone shaping with ear cartilage and tissue reinforcement. Incisions are made at the nasal tip and behind the ear (2-3 cm), enhancing nasal tip projection, dimension, and a natural appearance.

Rib Cartilage Rhinoplasty Technique

Rib Cartilage Rhinoplasty Technique effectively reshapes and enhances nasal structure. Ideal for those with multiple rhinoplasty revisions. Perfect for long-lasting, one-time solutions. The rib cartilage offers superior strength.Delivers a natural-looking nose shape.Highly durable and ensures maximum safety over time.

Rib Cartilage Structural Rhinoplasty

Operation details

  1. Small incision of only 1.5 – 3 cm under the breast.
  2. Uses rib cartilage from the 5th, 6th, or 7th rib.
  3. Takes approximately 6 hours, depending on individual complexity.

Why Rib Cartilage Structural Rhinoplasty is the Best Choice?

  • Achieves a more pronounced and elegant nasal tip compared to other techniques.
  • Utilizes the patient’s own cartilage, reducing risks of rejection or complications.
  • Provides superior strength and durability for long-lasting results.

Summary of the advantages and disadvantages of rib cartilage rhinoplasty

Rib Cartilage Rhinoplasty VS Ear Cartilage Rhinoplasty

Rib Cartilage Rhinoplasty

2.Requires general anesthesia.

3.Leaves a surgical scar at the rib site.

4.May cause temporary numbness at the rib site.

Ear Cartilage Rhinoplasty

Suitability : Suitable for those with adequate nasal tissue, non-thin nasal tips, or requiring minor tip adjustments for projection, refinement, or correcting asymmetry.

Recovery Time : Approximately 1-2 weeks

AdvantagesDisadvantages

1.Smaller surgical incision.

2.Faster recovery time.

3.No additional rib incision required.

1.Limited ear cartilage may be insufficient for certain cases.
